CASE COORDINATION PROGRAM (CCP) & COMMUNITY SUPPORT TEAM (CST) 

The Case Coordination Program (CCP) & Community Support Team (CST) are community-based programs for adults who have emotional/behavioral concerns and need a variety of services to achieve community stabilization. 

The Case Coordination Program (CCP) provides services to adults, age 18 +, who have a history of frequent psychiatric hospitalizations and require intensive case management in order to remain stable within the community. 

  • Treatments are guided by individualized treatment plans designed to meet the unique personal and cultural needs of each consumer.

  • BEW’s Case Coordination program links individual consumers with necessary services and social support within the least restrictive clinically appropriate settings.

  • Case Managers maintain supportive contact with the consumer to help consumers resolve practical problems.

 Case Coordination services include:

  • Direct access to Psychiatric support

  • Medication management

  • Socialization and support groups

  • Psycho-education groups focused on improving the consumer’s ability to self-manage mental health

  • Life skills group

 

The Community Support Team (CST) works with each consumer to:

  • Prevent frequent hospitalizations

  • Achieve stability in the community

  • Manage presenting symptoms utilizing the consumer’s strengths

Through a team approach CST serves every consumer on an as needed basis at BEW, in the community, or at consumer’s place of residence. CST works with each consumer to complete a plan of recovery designed to help them achieve community stabilization.  Each recovery plan includes:

  • Referrals

  • Psychotropic Medication Monitoring and Training

  • Daily living skills training (cooking, cleaning, washing, budgeting, managing money and shopping)
